Supporting Documents

Claims will only be paid when invoices and supporting documents are submitted.
Scanned copies of invoices are acceptable, and can accompany this claim form, preferably as one file. However, the originals must be retained for reference.
Should there not be enough space on the claim form, please type the information on a separate sheet and attach the file to this form.

Emailing Form

Once the claim is complete, clicking the submit button will email the form and attachments to KJM and send you a confirmation copy.
